Low back pain is a multidimensional, complex and often enigmatic affliction, it is a massive problem and extremely costly on many economic and societal levels. LBP affects everyone, whether you have suffered it yourself, know someone who has, treat LBP clinically to make a living, or pay for LBP directly or indirectly via your job, your region’s healthcare system, or taxes.
To illustrate, from 1994 to 2007 MRIs have increased 307%, spinal fusion surgery has increased 204%, spinal injections have increased 629%, and opiate use has increased 423%. To put this in perspective, during this time period the corresponding patient population increased by only 12%! Despite these alarming numbers, patient outcomes are worsening and the consequences of LBP are growing ominously on many fronts, affecting ALL of us. What is it that makes diagnosis and treatment so difficult? There are many structures that can contribute to low back pain. These include: the muscles of the low back, joints as known as facets, the discs, nerves, and vascular structures. There are also imbalances and instabilities that can lead to these structures being overloaded over time. Imaging of these structures is often unreliable. Studies suggest false positives increase with age. One such study looked at a large group of people with no LBP and MRI findings. The ages of the patients ranged from 30-60 both men and women. The results showed that approximately 30% of people at the age of 30 have disc herniation that would be considered clinically operable, 40% of people at the age of 40 had a disc herniation that would be considered clinically operable, with no signs and symptoms and the percentages continue to rise according to the increase in age. So what is the answer to assessing and treating low back pain. What most people in manual medicine world found is that there is no one treatment that's a magic bullet for low back pain. The important part is in assessing low back pain is a thorough history, neurological exam, and a full functional screening that would determine what is ultimately driving the low back pain. In many cases low back pain is coming from somewhere else in the body; some dysfunction in the foot, hip, pelvis and into the mid-back area. Sometimes there are conditions that create low back pain due to repetitive movements that will not be fixed with any surgery or manual therapy until that repeated movement has been altered or stopped. What I found in the past 10 years in treating low back pain is that most of the problems and dysfunction lie below the low back or lumbar spine. The lumbar spine is forced to do tasks it can't perform over long periods or it is forced to move in directions that it's not meant to move in. This overload due to a lack of motion or instability further down the chain results in disc herniation, irritated joints, and overworked muscles. It is very likely that treatments should be directed at controlling the symptoms but the focus needs to be paid to those areas that are not working correctly that would lead to overload of the lumbar spine of the low back. There is no magic bullet or magic treatment for low back pain. Many times it's a combination of treatments that will help with the muscle issues, the joint issues, and the rehabilitation of those areas that are working correctly.
